What is Mini-Screw Assisted Expansion?
MARPE is an orthodontic treatment that uses a palatal expander secured by small titanium miniscrews. The miniscrews anchor the appliance to the bone in the roof of the mouth, allowing expansion forces to act more directly on the upper jaw.
A traditional expander relies primarily on the teeth for support and is often effective in children whose jaws are still developing. MARPE provides additional skeletal support for patients whose palatal bones have matured.
When MARPE May Be Recommended
MARPE may be used to address:
- A posterior crossbite
- A narrow or high-arched palate
- Crowding related to limited upper jaw width
- A width difference between the upper and lower jaws
- Excessive outward tipping of the upper back teeth
- A previous expansion attempt that did not produce adequate widening
Although this treatment is commonly used for older teens and adults, age is not the only consideration. Palatal anatomy, bone quality, dental health, and the amount of expansion required also affect whether the treatment is appropriate.
MARPE Treatment Process
Our role in MARPE treatment focuses on planning and placing the miniscrews that provide skeletal support for the orthodontic appliance.
- Evaluation and planning: Dr. Exstrom examines the palate and reviews the orthodontist’s treatment plan. When appropriate, CBCT imaging is used to evaluate the available bone and plan safe, stable miniscrew positions.
- Miniscrew placement: Small titanium miniscrews are placed into the palate according to the coordinated treatment plan. Anesthesia and sedation options are tailored to the patient’s needs, preferences, and medical history.
- Follow-up and healing: We monitor the surgical sites as needed and address any concerns related to the miniscrews or healing.
After placement, the orthodontist provides instructions for activating and caring for the appliance and manages the expansion and orthodontic treatment.
Recovery and Healing
Mild soreness or tenderness may occur after miniscrew placement and typically improves within a few days. Most patients can return to their normal routine quickly.
Clear post-operative instructions are provided for keeping the area clean, managing discomfort, and supporting proper healing.
Dental Insurance and Payment Options
Insurance coverage varies by plan and by patient. We are in-network with the majority of insurance providers to help estimate available benefits, but coverage verification does not guarantee payment. Treatment fees are reviewed during your consultation so you have a clear understanding of expected costs.
FAQs
Is MARPE placement painful?
The palate is numbed before miniscrew placement, so patients typically feel pressure rather than sharp pain during the procedure. Mild soreness or tenderness afterward usually improves within a few days.
How long does the appliance stay in?
The active expansion phase may take several weeks, but the appliance usually remains in place for several months afterward. This stabilization period allows new bone to form in the expanded area. The orthodontist determines when the appliance is ready to be removed.
Will MARPE create a gap between my front teeth?
A temporary gap may appear between the upper front teeth as the two sides of the upper jaw separate. This can be a normal sign that skeletal expansion is occurring. The orthodontist closes or manages the space during the next stage of treatment.
Will MARPE widen my face?
Widening the upper jaw may produce subtle changes in the smile or facial proportions, but MARPE is not primarily a cosmetic procedure. The amount of visible change varies and depends on facial anatomy, the degree of expansion, and the overall orthodontic plan.
Can MARPE improve nasal breathing?
Upper jaw expansion can increase space within parts of the nasal cavity, and some patients notice easier nasal breathing. However, breathing changes cannot be guaranteed. MARPE is not a stand-alone treatment for sleep apnea or another diagnosed breathing disorder.
