First Visit

Thank you for choosing Offshore Oral & Facial Surgery for your surgical needs. Your initial appointment will consist of a consultation explaining your diagnosis and discussing all treatment options. If new or additional x-rays are required for treatment planning, they will be taken at this appointment.

We kindly ask you to arrive 15 minutes prior to your consultation appointment time to check in.

Please assist us by bringing the following items to your consultation visit: o Please print and complete the Patient Registration Form.

• Medical and dental insurance cards
• Photo ID
• Referral form (given to you by your dentist)
• Any relevant x-rays (given to you by your dentist)
• A list of medications you are presently taking.

Providing these items will expedite your visit and allow us to provide you with the best possible care.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian.
